Director of Care Management

Centegra Health System, a nationally recognized organization for clinical quality and service, seeks an accomplished nurse leader to serve as Director of Care Management. Reporting to the Vice President, Clinical Effectiveness, the Director of Care Management is responsible for leadership, development and implementation of strategic initiatives supported by care coordination across the continuum. The Director is familiar with, and facilitates compliance of, regulatory requirements related to care management and clinical documentation, and understands the clinical documentation needs of payers and organizations in the continuum of care in meeting the patient's needs during and post discharge from acute care.
Position requires Bachelor's degree in nursing, certification in case management, a minimum of five years successful management experience and eligibility for Illinois state licensure.
Master's degree is highly preferred. Candidates must be knowledgeable in all aspects of care management as it relates to the continuum of care. Excellent interpersonal communication and negotiation skills, strong organizational and time management skills, and ability to work independently and exercise clinical judgment in interactions with physicians, payers, patients and families are critical.
Centegra Health System has two hospitals, Centegra Hospital-Woodstock (145 beds) and McHenry (166 beds) and is located 50 miles northwest of Chicago. Besides the two hospitals, Centegra offers two immediate care centers, a cancer center and two fitness centers. The System has a Level II Trauma Center and a Level II nursery at each medical center.
